Welcome to Clean Street Diagnostics  These Terms and Conditions outline the rules & guidelines for using our services and website. By accessing or using our services, you agree to comply with these Terms. If you do not agree, please choose another service provider. Thankyou! 

 

1. Services Provided

 

Clean Street Diagnostics offers mobile phlebotomy services, which include the collection of blood and other specimens for diagnostic testing. We aim to provide a seamless, accurate, professional experience for our clients in their dedicated testing spaces and workplaces. However, please note:

  • Service Limitations: Services are subject to availability, and Clean Street Diagnostics  reserves the right to refuse or modify services at any time without prior notice.

  • Healthcare Disclaimer: Our services are intended solely for diagnostic purposes and are not a substitute for medical advice, diagnosis, or treatment. Always consult a licensed healthcare provider for medical concerns.

  • We do NOT offer in home testing. 

  • Clean Street Diagnostics has partnerships with LabCorp, DDC’s PeekaBoo, Gateway Genomics SneakPeek.

2. Eligibility and Client Responsibilities

 

By using our services, you confirm that you are at least 18 years old or have the consent of a legal guardian. You also agree to:

  • Provide Accurate Information: Ensure all information provided is accurate, current, and complete for proper service delivery.

  • Follow Health and Safety Guidelines: Comply with any health and safety guidelines provided by our staff during the visit to ensure the safety of both clients and staff members.

  • Payment Obligations: Agree to pay all fees associated with the services rendered, including any additional charges for services outside the initial scope.

3. Appointments and Cancellations

  • Appointment Scheduling: Appointments can be scheduled by contacting our office. Please ensure availability at the scheduled time to avoid delays.

  • Cancellation Policy: We require [e.g., 24 hours] notice for cancellations or rescheduling to avoid fees. Failure to provide adequate notice may result in a cancellation fee.

  • Late or Missed Appointments: If a client is not present or accessible within 15 minutes(grace period) of the scheduled time, we may consider the appointment missed and may charge a rescheduling fee.

4. Payment Terms

  • Service Fees: Clean Street Diagnostics provides transparent pricing for services. All fees are due at the time of service unless otherwise arranged.

  • Billing and Payment: We accept multiple payment methods, including credit/debit cards VISA, Master card, AMEX etc. along with electronic payments. By providing payment information, you authorize Clean Street Diagnostics to charge your chosen payment method. We do NOT accept cash or checks. 

  • Non-Refundable Fees: In general, payments for completed services are non-refundable.

5. Privacy and Data Protection

6. Intellectual Property

 

All content, materials, and branding on our website and associated materials, including text, graphics, logos, and images, are owned by Clean Street Diagnostics or licensed to us. Unauthorized reproduction, modification, or distribution of any content is prohibited without express permission.

 

7. Limitation of Liability

 

To the extent permitted by law, Clean Street Diagnostics shall not be liable for any direct, indirect, incidental, special, or consequential damages resulting from your use of our services, including but not limited to:

  • Service Interruptions or Delays: Clean Street Diagnostics is not responsible for delays or interruptions in service caused by unforeseen events beyond our control, such as natural disasters or technical issues.

  • Results and Outcomes: Clean Street Diagnostics provides accurate and timely diagnostics, but we do not guarantee specific health outcomes or interpretations of results. Any diagnosis or treatment should be provided by a licensed healthcare provider.

You agree to indemnify and hold Clean Street Diagnostics, its affiliates, employees, and agents harmless from any claims, damages, liabilities, losses, or expenses, including reasonable attorneys’ fees, arising out of or related to your use of our services or any violation of these Terms.

Clean Street Diagnostics reserves the right to modify or update these Terms at any time. Any changes will be effective upon posting, and continued use of our services indicates acceptance of the revised Terms. We encourage you to review these Terms periodically for updates.

These Terms and Conditions are governed by the laws of Ohio, and any disputes arising out of or relating to these Terms shall be resolved in the courts of Ohio.
